• Your diagnosis for cancer or other tumor types
• Imaging or diagnostic records from your physician
• Information about any prior cancer treatment you may have had
• Pathology
• Family medical history
• Your current medications
• Allergies
• Insurance information
• Availability for treatment
• A referral for radiation therapy
